How to Download and Fill Out BIR Form 1905

Step 2: Fill Out BIR Form 1905

Part I: Taxpayer Information

Check the box that corresponds to your civil status: single, married, widow/widower, or legally separated.

Part II: Reason/Details of Registration Information Update/Correction
Check the box that corresponds to the reason for filing the form: updating/correction of registration information, cancellation of registration/TIN, new copy of TIN card/certificate of registration, or others (specify).
If you are updating or correcting your registration information, check the box that corresponds to the information that you want to change: tax type details, taxpayer details, registered activities/line of industry/business, accounting period/ method/manner of recording books of accounts/inventory system/other accounting policies/practices.
If you are canceling your registration or TIN, check the box that corresponds to the reason for cancellation: closure/cessation of business/employment/practice of profession/dissolution/merger/consolidation/death/others (specify).
If you are requesting for a new copy of your TIN card or certificate of registration, check the box that corresponds to the reason for request: lost/stolen/damaged/others (specify).
Fill out the table with the details of your current and new information, as applicable. Use additional sheets if necessary.
Part III: Declaration
Read and understand the declaration statement at the bottom of the form. Sign and date the form in the space provided.
If you are filing on behalf of a corporation or partnership, affix your official capacity and corporate seal.
Step 3: Submit BIR Form 1905
After filling out BIR Form 1905, you need to submit it to the RDO where you are registered. You also need to attach the following documentary requirements:
Notice of closure or cessation of business;
Notice of death and death certificate, in case of death of an individual;
